How the Referral Process Works
We’ve streamlined our referral process to make it fast, simple, and reliable for our professional partners.
Submit Referral
Send a preliminary referral request with the form below, or call the office.
Discussing Needs
The office reviews the request during office hours and contacts you, then the client or family, to discuss needs. We respond to new inquiries within one business day.
In-Home Assessment
With the client’s agreement, a trained staff member visits the home to understand routines, safety, and the support needed.
Care Plan Developed
A written, non-medical plan of care is developed with the client and family, and approved by the client or their representative.
Arranging the First Visit
We discuss caregiver availability and arrangements for the first visit.

What happens next
- The office reviews requests during office hours (Monday to Friday, 8 AM to 5 PM) and contacts you using your preferred method.
- With the client's agreement, we contact them or their family to discuss needs and options.
- If any further information is needed, we agree a suitable way to share it. This website form is not a system for sending medical records.
